NOTICE OF FORMATION OF A DOMESTIC LIMITED LIABILITY COMPANY

INTRAFORM LLC has been formed as a domestic limited liability company (LLC) by filing Articles of Organization with the New York Secretary of State on July 29, 2026. The office of the LLC is located in Erie County, New York. The New York Secretary of State has been designated as agent of the LLC upon whom process against it may be served. The Secretary of State shall mail a copy of any process served upon the LLC to INTRAFORM LLC, 3279 Walden Ave., Depew, NY 14043. The purpose of the LLC is to engage in any lawful act or activity for which a limited liability company may be organized under the laws of the State of New York.
